Release of Patient Information

Health Information Management is the centralized location for release of patient information. Patients needing patient information should go to Room 120 on the 1st floor of Somerset Hospital.

Please note, for convenience, patients may use the door located near the Human Resources Department on Church Street between the hours of 7:00 a.m. and 5:00 p.m., Monday through Friday.  Patients may park in the lot across from Human Resources for easy access to the Health Information Management Department.  The telephone number for Health Information Management is 814-443-5154.

An authorization form for the release of medical information must be completed for all requests.  You may click here for a copy of the authorization form.  Patients may also request records via the internet by contacting records@somersethospital.com
