WHO WE ARE
In Touch Hospice is a Medicare-certified nonprofit organization dedicated to bringing comfort into the home at this most distressing time. While able to access Medicare and most private insurances, In Touch Hospice has a long history of providing charitable care as a community service. Our office is located in the Hospice House at 1474 North Center Avenue in Somerset. The Hospice House provides inpatient, respite and residential care for hospice patients. WHAT IS HOSPICE?
The concept of hospice shows a concern for the total patient and their family which makes hospice care very different from traditional forms of health care.
WHAT SERVICES ARE PROVIDED BY
SOMERSET HOSPITAL’S IN TOUCH HOSPICE?
-Regular visits by nurses trained in the hospice 24 hours a day
-Consultation and cooperation with the patient’s physician or the hospice medical director to
provide pain and symptom control
-Provision for the patient’s personal care needs
-Any durable medical equipment and supplies needed to ensure comfort and quality of life
-Medications related to terminal illness
-Visitation by a social worker to assist with meeting the patient and family’s psychosocial
and emotional needs
-Visitation by the hospice chaplain to assist with the patient and family’s spiritual needs
-Volunteers who become meaningful friends, who are willing to listen, share and
help ease the problems of the terminal illness.
The hospice concept maintains the belief that its care does not end with death. Contact continues with the family during the bereavement period.
-Monthly mailing of the "Journeys" newsletter for the bereaved
-Monthly support group
-Camp KiDD support group for children aged 6-14 years of age who are bereaved
-An annual memorial service
