Release of Patient Information is located on the first floor of UPMC Somerset, near the main entrance.
To get to the department, enter the hospital at the main entrance. Walk past the registration desk and down the hall. Once you pass the silver elevators and can’t go any further, make a right and the office will be on the left side of the hallway. The department will be providing services between the hours of 8:00 a.m. to 4:30 p.m., Monday through Friday. Patients may park in the parking lot at the main entrance. The telephone number for obtaining patient records is (814) 443-5158.
An authorization form for the release of medical information must be completed for all requests. Download a copy of the authorization form. Patients may also request records via the internet by contacting email@example.com

UPMC Somerset offers patients over the age of 18 the opportunity to access health information online and on the go.
To protect your health information, sign-up must be completed in person, in which you will need a valid form of identification and email address. You may request access to the portal at your next visit, or stop in to the main entrance of UPMC Somerset and see the registrar at the front desk. Once your identity has been verified, UPMC Somerset will send you an invitation email with link and instructions to set up an account. The sign-up sheet below may be printed off and brought in with you.
